Agreement

By accessing FORMA services, you agree to these Terms. If you do not agree, do not use the services. If you use FORMA on behalf of an organization, you represent that you have authority to bind that organization.

Eligibility

You must be at least 18 years old (or the age of majority in your jurisdiction) to create an account. Parental controls are available for family profiles managed by a primary account holder.

Service description

FORMA provides home 3D body scanning, measurement estimates, and wellness-oriented analytics. The service is not a medical device and does not provide medical advice. Always consult qualified professionals for health decisions.

Accounts and security

You are responsible for safeguarding credentials and for activity under your account. Notify us promptly of unauthorized access at [email protected].

Subscriptions and hardware

Paid plans renew according to the billing cycle selected at purchase unless canceled. Hardware is subject to separate warranty terms included with shipment. Refund eligibility is described at checkout and in support documentation.

Acceptable use

• Do not reverse engineer, scrape, or abuse APIs beyond documented limits.
• Do not upload content you lack rights to or that violates others' privacy.
• Do not misrepresent FORMA outputs as clinical diagnoses or HIPAA-covered records for consumer accounts.

Intellectual property

FORMA software, models, and branding remain our property or our licensors'. You retain rights in your scan data subject to the license needed to operate the service.

Disclaimers

Services are provided “as is” to the extent permitted by law. We disclaim warranties of merchantability, fitness for a particular purpose, and non-infringement. Measurement accuracy varies; see product documentation.

Limitation of liability

To the maximum extent permitted by law, FORMA is not liable for indirect, incidental, or consequential damages. Our aggregate liability for consumer accounts is limited to amounts paid in the twelve months preceding the claim.

Changes

We may update these Terms. Material changes will be communicated via email or in-app notice. Continued use after the effective date constitutes acceptance.
