Overview

FORMA (“FORMA,” “we,” “us”) operates the FORMA home 3D body scanner platform. This policy describes how we handle personal information when you use our website, hardware, mobile apps, and cloud services.

Information we collect

• Account details: name, email, password hash, billing information via our payment processor.
• Scan data: images, depth frames, reconstructed meshes, derived measurements, and timestamps.
• Device and usage data: app version, error logs, and anonymized performance metrics.
• Communications: support tickets and contact form submissions.

How we use information

We use your data to provide scan reconstruction, measurement history, AI-generated insights, customer support, and product improvement. We do not sell personal scan data to advertisers. Marketing emails are opt-in and include unsubscribe links.

Sharing

You control sharing with family members and coaches via explicit invitations. We use subprocessors for cloud hosting, payment processing, and email delivery under contractual data-protection terms. We may disclose information when required by law or to protect safety and rights.

Security

We apply TLS for data in transit and AES-256 encryption at rest for stored scan artifacts. Access is role-based and logged for business accounts. No method of transmission or storage is 100% secure; we continuously review our controls.

HIPAA and health information

Standard consumer FORMA accounts are not HIPAA-covered services. We do not represent that the consumer product is HIPAA compliant. Enterprise customers that are covered entities may enter separate agreements that can include Business Associate Agreements (BAAs) and additional safeguards. Medical disclaimer

FORMA is a wellness and fitness measurement tool. It is not a medical device and is not intended to diagnose, treat, cure, or prevent any disease. Outputs are estimates with confidence ranges—not clinical laboratory results.

Your choices

• Access, export, or delete scan data from account settings.
• Revoke coach or family sharing at any time.
• Request account deletion by emailing [email protected].

International users

Data may be processed in the United States and other regions where our subprocessors operate. Where required, we provide appropriate transfer mechanisms.
